A payroll agent, sometimes referred to as a fiscal agent, is an agency who bills Medicaid for the services the individual receives. A payroll agent also performs payroll as well as required tax and insurance withholding functions for others. They pay the provider's employee(s) and ensure the employee(s) are paid according to the documented hours worked.

A self direct option may be chosen for supported home care or respite care by the individual or, if the person is incapable of providing self-direction, by the individual's guardian or parent acting on their behalf. If a provider is hired and the work is being directed by a beneficiary's family member or the beneficiary him/herself, then billings should be made through the CDDO or an agent that is affiliated with the CDDO that will account for all necessary deductions including: taxes, unemployment, and worker's compensation, as applicable.
